      The sports school accepts ALL children who wish to play football and who have fulfilled the standards established by the football, futsal curriculum. and have no medical contraindications.
 
         Зарахування до Комунального закладу ДЮСШ " "Futsal Academy" of the Zhytomyr City Council is carried out on the basis of:
 
         1. Statements of parents or persons replacing them;
 
         2. Medical opinion of the doctor of the polyclinic at the place of residence that there are no medical contraindications for classes;
 
         3. Copies of birth certificate and identification code.
